I have a Promo copy, white label of this record.A runout = CR-151 A and CR-154 B. Has one nick on the Good Vibrations side that doesn't affect play. Overall sound is not mixed well and fades in and out. Other than that it looks like it just came off the press.

Just got a regular copy of this one. The fading bit you mentioned is especially true for the flipside, which fades in at the start.
